Parent/Guardian Waivers

GVYF seeks to instill positive character-building traits in our community’s youth through the demonstration of good sportsmanship, respect for others, responsibility, fairness, caring and good citizenship. We ask that parents and their guests attending GVYF sponsored events help us by reflecting these character traits at games.

When attending GVYF sponsored events, I therefore agree:

• I will be a positive role model for my child and encourage sportsmanship by showing respect and courtesy, and by demonstrating positive support for all players, coaches, officials and spectators at every game, practice or other sporting event.
• I will not engage in any kind of unsportsmanlike conduct with any official, coach, player, GVYF Board member, or parent such as booing, taunting or using profane language or gestures, etc.
• I will not encourage any behaviors or practices that would endanger the health and wellbeing of the athletes.
• I will teach my child to play by the spirit of the rules and to resolve conflicts without resorting to hostility or violence.
• I will demand that my child treat other players, coaches, officials and spectators with respect regardless of race, sex, or ability.
• I will never ridicule or yell at my child or other participant for making a mistake or losing a competition.
• I will respect the officials and their authority during games and will never question, discuss, or confront coaches, referees, or GVYF Board members at the game field, and will take time to speak with coaches at an agreed upon time and place.
• I will refrain from coaching my child or other players during games and practices, unless I am one of the official coaches of the team.
• I will inform the coach of any physical disability or ailment that may affect the safety of my child or the safety of others.

I also agree that if I fail to abide by the aforementioned rules and guidelines, I will be subject to disciplinary action that could include, but is not limited to the following:

• Verbal warning by official, head coach, and/or member of league organization
• Written warning
• Parental game suspension with written documentation of incident
• Parental season suspension

I also hereby consent, without further consideration or compensation, to the use (full/partial) of all photos, audios or videos made of my child or family members during GVYF sporting events, for the purposes of event documentation, promotional materials or web content. Further, I release GVYF and its volunteers from any liability, which may arise from the use of those photos. 

I/we understand that no parent, guardian or relative of the child registered above is permitted on the practice or game field during the course of the games or practices without proper security clearances and approval from Grand Valley Youth Football. By initialing below, I also acknowledge I have received the GVYF Parent Code of Conduct and agree to the Conduct’s terms.

I/we guarantee the return of all equipment loaned to our child and accept responsibility for the loss or damage of said equipment. Failure to comply with timely equipment return will result in GVYF pursuing all legal channels to recover equipment or replacement costs.

The registration cost for the 2024 Fall Tackle season is $200 check, cash, or card. The registration cost for the 2024 Flag Fall season is $100 check, cash, or card. 

All registration fees are non-refundable once practice starts.

$50.00 returned check fee will apply. 

*A copy of the player's birth certificate is required to be eligible to participate.

**Proof of school of attendance (report card, ParentVue) and/or proof of physical mailing address (utility bill) is required to be submitted by 8/1/2024 to reserve your roster spot.

Your child WILL NOT receive their GAME DAY UNIFORM if not PAID IN FULL. NO EXCEPTIONS!

In consideration of my/our child being allowed to participate in any way for the Grand Valley Youth Football Program, its related events and activities, the undersigned, acknowledges, appreciates, and agrees that: 

1. The risk of injury from the activities involved in this program is significant, including the potential risk for permanent paralysis and death, and while particular rules, equipment and personal discipline may reduce the risk, the risk of serious injury does exist; and
2. I knowingly and freely assume all such risks, both known and unknown, even if arising from the negligence of the releases or others, and assume full responsibility for my participation; and, 
3. I willingly agree to comply with the stated and customary terms and conditions for participation. If, however, I observe an unusual and significant hazard during my presence or participation, I will bring such to the attention of the nearest official immediately; and
4. I, for myself and on behalf of my heirs, assigns, personal representatives and next of kin, herby release, indemnify, and hold harmless Grand Valley Youth Football, its officers, officials, agents and/or volunteers, other participants, sponsoring agencies, sponsors, advertisers, and if applicable, owners and lessors of premises used to conduct the event, with respect to any and all injury, disability, death, or loss or damage to person or property, whether arising from the negligence of the releases or otherwise, to the fullest extent permitted by law.
5. If a child leaves a practice or game for any medical reason, he or she cannot return without medical clearance from a physician.
6. At any time, a coach can require medical clearance to be able to participate or play at his/her own discretion.

I HAVE READ THIS RELEASE OF LIABILITY AND ASSUMPTION OF RISK AGREEMENT. I FULLY UNDERSTAND ITS TERMS. I UNDERSTAND THAT I HAVE GIVEN UP SUBSTANTIAL RIGHTS BY SIGNING IT, AND SIGN IT FREELY AND VOLUNTARILY WITHOUT ANY INDUCEMENT.
